DVLA Update: What HGV and Bus Drivers Need to Know About D2/D4 Forms

The DVLA has made a key change for professional drivers: as of 15 September 2025, the online Form Filler service for D2 and D2/D4 vocational licence applications has been withdrawn. This affects how you get the paper forms needed to apply for or renew your HGV or bus/coach licence.

  • You can no longer request D2 or D2/D4 forms online.
  • Paper forms are still available at the Post Office.
  • The DVLA will send you renewal reminders 56 days before your licence expires, including the forms you need.

Forms You Need to Know

D2 – Application for a Vocational Licence

  • Use it to apply for or renew licences for HGVs (LGVs) and PCVs (buses/coaches).

D4 – Medical Examination Report

  3. Book a medical check and have your doctor complete D4.
